Abstract:	現今市面上以及網路上常可以看到琳瑯滿目的問卷系統，有些甚至複雜到令人難以使用，所以我們利用了PHP語言結合了SQL資料庫完成了這套問卷系統，我們盡可能做到最人性化的介面，讓每位使用者用起來可以更加順手，不讓複雜的介面帶給使用者許多的不方便，也讓管理者可以方便的控管使用者以及各項問卷的資訊。在管理者方面，管理者本身可以看到所有問卷以及題目的資訊，另外還有新增、修改、刪除問卷的功能。在管理者的系統管理方面，可以對使用者進行檢視的動作。
